Local Bodybuilder Comes Back From Torn Bicep to Win Mr. Indiana

After a torn biceps tendon threatened his bodybuilding goals in June 2025, Joshua Smith returned to competition and earned the Mr. Indiana 2026 title in August following surgery performed by Eric P. Tannenbaum, M.D., of Southern Indiana Orthopedics.

Smith tore his biceps tendon on June 4, 2025, just nine weeks before the competition he had been working toward for years. He began working with Dr. Tannenbaum in mid-June and underwent distal biceps tendon repair surgery. The injury and recovery forced Smith to shift his focus from competition to rehabilitation and rebuilding his strength.

Following surgery and months of rehabilitation, Smith gradually returned to lifting and training. On Aug. 1, 2026, he achieved his long-standing goal of winning Mr. Indiana at the NPC Premier Muscle & Indiana State Championships. He also earned an overall championship title at the event.

A distal biceps tendon injury affects the tendon that attaches the biceps muscle to the forearm near the elbow. Repair surgery aims to reattach the injured tendon, while rehabilitation helps patients work toward restoring motion and strength. The timeline for returning to heavy lifting depends on the injury, treatment and individual recovery.

“Recovering from a distal biceps tendon injury takes time, dedication and a commitment to rehabilitation,” said Dr. Tannenbaum. “Joshua worked hard to return to the sport he loves, and seeing him achieve his goal of winning Mr. Indiana is incredibly rewarding. His story is a great example of what determination and the right treatment plan can accomplish.”

Smith’s comeback offers an example of the determination involved in recovering from a significant injury and returning to a demanding sport. Southern Indiana Orthopedics provides orthopedic care for patients seeking treatment for injuries and conditions affecting their ability to stay active.
